Erythema nodosum is an inflammation of subcutaneous fat tissue. If the inflammation is pressed it causes quite a lot of pain. It appears to be a painful bruise on both the shins. Other bumps may be seen on the elbows, knees, thighs and forearms. The person affected by Erythema nodosum may feel as if suffering from flu. Women are 3 times more likely to be affected by Erythema nodosum. People in the age group of 15 to 30 years are more likely to get Erythema nodosum.
Cause
There are several causes of Erythema nodosum. The most common cause being medication, especially some brands of contraceptive pills and sulfa drugs. On changing the medication Erythema nodosum resolves on its own. Another major cause of Erythema nodosum is infection. Viral infections, Yersinia enterocolitica, Mycoplasma pneumoniae, Leprosy, Leprosy, Leprosy, fungal infection, bacterial infection and tuberculosis are a few of the other major causes.
A lesion biopsy is the first step in determining the cause. The doctor would try to determine the cause; this would involve skin tests and lab work. The condition is not contagious or dangerous. It disappears with the cause being removed.
Symptoms
The lesion begins as slightly raised and is bright red. Erythema nodosum ranges in size from one half inch to several inches. The color turns purplish red. As it starts to fade it turns to a yellowish color. The symptoms of Erythema nodosum need to be differentiated from bruising, phlebitis, insect bites, and pancreatitis.
Treatment
The most effective treatment is to remove the underlying cause. In case no cause is found a complete resolution would take 4 to six weeks. Indocin which is an aspirin like drug helps to improve the condition. The medication should be taken 3 times a day along with food or Maalox. For large and painful nodules injections may be used. Bed rest along with Non-steroidal anti-inflammatory can be used to reduce swelling and pain. A warm soapy wash cloth can be used to provide gentle heat. This process should be repeated 3 to 4 times a day for about thirty minutes to speed up the healing process. Oral potassium iodide is an effective medication but it is unpleasant to take and should be dosed correctly. In case of severe cases other treatments are available. In acute stages, if the swelling and pain is significant the patient’s mobility is restricted. Restriction of physical activities can help shorten the course of Erythema nodosum.
